What this service covers
Payers approve bills from providers they have never seen. Most are exactly what they appear to be. Occasionally an address on a bill turns out to be a mailbox store, a vacant suite or a residence, or a clinic is open two mornings a week while billing for five full days. A verification visit settles the basic question of existence and operation with photographs, and it takes one visit to one address.
This is a service for payers and their representatives and is not offered to claimants or to providers seeking a reference. The investigator records what a member of the public would see by walking up to the door during business hours. Delator does not evaluate credentials, quality of care or billing, does not enter treatment areas, and does not request patient information. The report does not characterize a provider as legitimate or illegitimate.
Reasons to verify a provider on site
- A provider new to the carrier's system begins submitting a large volume of bills from an address nobody recognizes.
- Mail sent to a clinic's billing address is returned, or its phone number rings to voicemail at all hours.
- A provider lists several practice locations across different Tennessee counties and the reviewer wants to know which ones are staffed.
- The address on file maps to a strip center, an office park or a house, and satellite imagery does not settle what is there.
- A network or panel administrator wants periodic confirmation that listed providers are still operating where the directory says.
What a verification visit records

- Photographs of the building, suite number and surroundings, with a note on whether it is a medical office, retail unit, shared workspace, residence or vacant space.
- Signage and directory listing
- Exterior signs, door lettering and any lobby directory entry, recorded exactly as worded, including other names displayed on the same door.
- Posted hours versus actual
- Hours shown on the door or website, and whether the office was open, locked or unattended at the time of each visit.
- Visible operation
- What can be seen from the public entrance or waiting area: a reception desk, staff, seating, and whether the space appears furnished and in use.
- Patient traffic count
- Number of people arriving and leaving during a defined observation window, noted from a public area without photographing faces or recording identities.
- Public licensing lookups
- Results of publicly searchable state license and business registration lookups for the provider and practice name, attached as retrieved with the retrieval date.

What one visit can show, and when to return
A single weekday visit during posted hours answers the threshold question: there is or is not a functioning office at this address under this name. For many referrals that is enough. The investigator photographs the approach, the entrance and the suite, notes the time, and checks the directory. If the door is locked during posted hours, that is recorded, along with any notice on the door and what neighboring tenants were willing to say.
One locked door proves little. Staff take lunch, practices close for training, and some specialists see patients at a location only on certain days. When a first visit raises a question, the client can authorize return visits at different times and days. The report then shows a pattern across dates instead of a single moment. Routine deployment places an investigator on site within one to two days anywhere in the state.
Staying in public space
A medical office is a sensitive place to observe. Delator's investigators stay where the public is invited: the parking area, the common hallway, and the waiting room if it is open to walk-in visitors. They do not pass the reception desk, photograph patients or paperwork, or question staff about who is treated there. If asked, the investigator identifies as a licensed private investigator and does not invent a reason for being there.
Traffic counts are kept as numbers and times. No attempt is made to identify the individuals counted, and vehicles are noted only when the client has asked about a specific transport service. The purpose is to give a reviewer a sense of scale, such as two arrivals in four hours or forty, that can be weighed against the volume in the client's own data. The weighing is done by the client.
This service is available to insurance carriers, self-insured entities, third-party administrators and the attorneys who represent them. Delator reports facts and observed indicators; decisions on coverage, liability, settlement and any fraud referral remain with the client.
It is not offered to policyholders or claimants, and Delator does not act as a public adjuster. Policyholders and claimants can request neutral scene documentation instead.
Common questions about this service
Who can order a provider or clinic verification?
Workers' compensation carriers, self-insured employers, third-party administrators, special investigations units and attorneys representing them. Delator does not offer this service to claimants, to competing providers or to members of the public. The client supplies the provider name and each address to be visited.
Does the provider know the visit took place?
Often not, since the investigator observes as any visitor would. The investigator does not conceal identity if asked and does not give a false reason for the visit. If the client prefers an announced visit with a request to speak to the office manager, that can be arranged instead.
Do you verify a provider's qualifications?
Delator attaches what public license and registration lookups show on the date searched. It does not evaluate training, competence, scope of practice or quality of care, and it does not state whether a provider is qualified to render any service billed.
How fast can several addresses be checked?
Addresses in or near Nashville, Memphis, Knoxville, Chattanooga, Clarksville, Murfreesboro and Franklin can usually be grouped into a single day. Locations elsewhere are scheduled by region.
